Street Outreach & Advocacy Peer Specialist
Full Time position, 37.5 hours per week; Starting salary is $39,000 with benefits

This position has been developed to provide peer delivered services to homeless individuals and families. This position will use one-on-one engagement to ensure that all vulnerable people in this target population have their housing, medical, behavioral health, environmental health and other identified needs addressed. This position will collaborate with CCHN Street Outreach Case managers and community partners such as: Cayuga County Mental Health, Cayuga County DSS, Nicks Ride 4 Life, etc. to help connect the individuals served with needed resources and services using a person-centered approach. This position reports to the Director of Community Housing and Street Outreach.

Work with Street Outreach and Advocacy staff to locate and assist target population in the City of Auburn and Cayuga County
Work on-on-one with individuals as needed to facilitate navigation of services and resources (care coordination, housing, employment, SNAP benefits, healthcare, etc.)
Distribute supplies and resources to target population (hygiene kits, non-perishable food, referral info, etc.)
Assist contacts with applications for DSS, housing, Lifeline phone, Identity documents, employment, etc.
Conduct regular wellness checks on homeless individuals and families in shelters and hotels, in partnership with DSS and other community partners.
Connect individuals with other peer services through Nicks Ride and Cayuga County Mental Health
Attend relevant community meetings
Work with program Director on other tasks as needed.

Associate’s degree in a health, human services or related field, High School degree or GED will be considered with relevant experience
Must have lived experience with homelessness
Must be able to work independently or in a team and be self-motivated, resourceful, organized and adaptable in carrying out job duties
Must have good oral and written communication skills
Must possess computer skills and be proficient in Office 365
Must have demonstrated ability to work with a large cross-section of people in a warm, non-judgmental manner
Knowledge of Motivational Interviewing and Harm Reduction skills
Must have acceptable physical and mental health to carry out the responsibilities, lift 25-50 pounds weekly.
Travel throughout Cayuga county is required; must possess a valid NYS Driver's License with driving record within agency policy and have access to a reliable vehicle.
